Headway Emotional Services provides comprehensive mental health therapy, counseling and classes to more than 11,000 families in our Golden Valley, Richfield and Hopkins locations each year. Originally called The Storefront Group, Headway was founded in 1970 by a group of suburban parents who wanted to help kids and families in crisis in their communities. Their mission, still true today, was to transform lives by providing hope, healing and empowerment through treatment, intervention and education.

As a Case Manager, you will have the opportunity to directly impact the lives of children and their families through our successful mental health programs. You will be part of a caring and highly regarded staff of professionals in our Hopkins, Minnesota location.

We are looking for a professional to coordinate mental health services for children and families. In this role, you will be responsible for coordinating, arranging, and facilitating services for children with significant mental health symptoms that impact their daily lives. The goal is to improve the overall functioning of the family and reduce the need for and reliance upon out of home placements and other services that are more intensive, restrictive, and costly. You will manage an average of 15 active clients per month and ensure the case management treatment plans and functional assessments are completed in accordance with organizational, DHS, insurance, and county guidelines.

Location: Hopkins 


  • Minimum of one face to face core contact with the child/child's family per month (15 contacts minimum).
  • Have weekly contact either face to face or via electronic means with the client/client's family
  • Develop an  IFCSP and complete a functional assessment with the child/child's family within 30 days of starting services 
  • Review, reassess, and monitor implementation of the care plan every 90 days.   Ensure that the services outlined in the client's care plan are actually received.
  • Provide safety and crisis planning.
  • Assist families in completing day treatment paperwork and application and attend Day Treatment determination meetings.
  • Assist families in transitioning in and from Day Treatment programming.
  • Participate in training in the use of community resources and services.
  • Assess and coordinate client access to and receipt of needed services.  Problem-solve around the access to these services if needed.
  • Assist in integrating and providing linkages between school and home and multiple agencies the client and family is working with (i.e. mental health agencies, juvenile corrections, child welfare agencies, etc.)  
  • Assist the client/family in obtaining other non-mental health benefits and services related to their health and disability as needed (i.e. health care, special education).
  • Assist the client or family in building community support and family support systems.
  • Provide consultation and guidance to school staff regarding client needs.
  • Act as an advocate on behalf of the client and family with the service system.
  • Work collaboratively with the Day Treatment team to support successful completion of Day Treatment and transition to supports after completion.
  • Complete paperwork accurately and within deadline.
  • Attend clinical consultation meetings, staff and other meetings, as directed by the supervisor or agency policy.
  • Follow and enforce all agency policies and procedures.
  • Perform other duties as assigned.




  • Bachelor's degree in one of the behavioral sciences including but not limited to: social work, psychology, sociology, criminal justice, or nursing from an accredited college or university
  • Skilled in the process of identifying and assessing a wide range of client needs
  • Knowledgeable about local community resources and how to utilize these resources for the benefit of the client
  • Job/internship experience and training in working with children and families
  • Skilled in monitoring service outcomes
  • Must have car and auto insurance
  • Ability to work to evening hours

Preferred but not required

  • Minimum of 2,000 hours of supervised experience in the delivery of mental health/social services to children and/or adolescents with an emotional disturbance
  • Experience working with children/families in their homes
  • Experience working with school systems
  • Experience working with children identified as having a severe emotional disturbance

Knowledge, Skills, and Abilities

  • Proficient knowledge and understanding of mental health issues and resources
  • General knowledge and understanding of case management services
  • Proven ability to establish and maintain positive rapport with a wide variety of clients and professionals
  • Demonstrated ability to identify and resolve problems independently in a variety of arenas with both clients and professionals within the community
  • Demonstrated excellent written and verbal communication skills
  • Demonstrated ability to collaborate and utilize the skills of program team and other staff
  • Proven ability to use client's strengths to promote positive change
  • Proficient computer skills in Word and Excel programs
  • Experience and training in identifying and assessing a wide range of children's issues and needs
  • Ability to manage time efficiently in order to ensure paperwork and billing are on time and completed correctly
  • High degree of organization and ability to work independently 
  • Local travel within the metro area to a variety of arenas, including in-home, in the community and in schools
  • Interact with a range of personalities and diverse individuals
