The Plus Case Coordinator will be responsible for the day to day oversight and management of assisting enrolled Plus students to achieve success in reaching educational goals. The Plus Case Coordinator will consult with the Plus Educational Team, System Navigator, Hennepin County Probation and Court Staff and parents to assure student success in the program. In addition the Plus Case Coordinator will assist with problem solving student participation/attendance, behavioral issues and assist with coordination of other supportive mental/chemical health referrals services. Facilitate transition to mainstream education settings as appropriate. Transport youth as needed.

Location:  Richfield South Education Center Alternative, Richfield, MN

Hours: Monday-Friday during school hours. 

This is a full time, 40 hour per week position year-round.

ESSENTIAL JOB FUNCTIONS

  1. Services to students and families
  • Provide advice, guidance and support to students, parents/guardians and other stakeholders
  • Promote positive social skills and appropriate pro-social behavior with referred students
  • Consult and collaborate effectively with parents/guardians, educational staff, administrators and other educational/community resources regarding students with identified concerns and issues
  • Monitor student academic performance, behavioral and attendance and assist with appropriate interventions in conjunction with the other ALC Plus team members
  • Assist and support students who are experiencing emotional, behavioral, or health related issues and/or students who have difficulty attending school on a consistent basis
  • Support, track and assist with problem solving issues associated with students participating/referred to the ALC Plus Program
  • Provide case coordination for students referred into the ALC Plus Program
  • Develop professional caring relationships with program participants and families
  • Participate in recreational and educational after school activities for students
  • Travel to settings where services are provided, primarily family, home, and school and community resources
  • Transport program participants to educational settings, court appearances and home if necessary
  • Provide an appropriate professional role model for families and encourage/support parents in the parenting of their children
  1. Paperwork and reporting
  • Collect and accurately interpret, utilize and report student data as required
  • Collect and document designated data and be able to present data in staff meetings. Prepare reports as required
  • Provide an appropriate professional role model for families and encourage/support parents in the parenting of their children
  • Keep necessary case notes documenting contact with program participant, family and collateral contacts
  • Assure program participant privacy is maintained by being knowledgeable and following HIPPA/FERPA data privacy and security regulations
  1. Resource identification and referral
  • Assist with the identification unmet social/emotional needs of students and make recommendations/referrals for further assessments and services
  • Introduce and refer students to resources that may assist them with personal and educational issues
  • Assist families in connection with community resources
  • Develop and utilize available area resources and make appropriate referrals
  1. School partnerships
  • Participate in the integrated/multi-disciplinary intake team to assist in the development of a complete Student Service Plan and other intake paperwork
  • Participate in school staff meetings and student conferences as needed
  1. Other
  • Attend on-going staff development opportunities as required by the contracting agency and comply with Intermediate District 287 Policy and Procedures including mandatory training

 

QUALIFICATIONS

Required

  • BA degree in Human Services or related field
  • 2+ years of experience in working with adolescents and families
  • Must have their own car for work
  • Able to work evening hours

Preferred but not required

  • Experience working within a school setting
  • Experience with juvenile justice system
  • Fluent in Spanish

Knowledge, Skills, and Abilities

  • Knowledge of Education and County Probation and Human Services Systems
  • Ability to communicate effectively, both orally and in writing
  • Ability to work with a diverse population of individuals by using tact diplomacy and flexibility
  • Skill in dealing with parent and student concerns
  • Skill in communicating with school educational staff, Hennepin County representatives
  • Ability to have a flexible schedule to consistently accommodate phone and email inquiries
  • Works under moderate supervision
  • Knowledge of adolescent mental and chemical health issues
  • Knowledge of Hennepin County Department of Corrections and Human services systems
  • Knowledge of Educational resources
  • Awareness of community based mental and chemical health resources
  • Ability for creative problem solving, decision making, independent judgment and negotiating/mediating
